We collect your IP address, browser and device type (user agent), the page address you visited, the date and time of your submission, and campaign identifiers included in the link that brought you to the Site. We also derive your city and state from your ZIP code.
Form session records. We use TrustedForm, a service provided by ActiveProspect, to create a record of your visit that documents what you saw on the page and the consent you gave. This record may include your interactions with the form, such as clicks and keystrokes.
Cookies and similar technologies. The Site and the services we use, such as TrustedForm, may use cookies and similar tools to operate the form and document your visit.
How we use information
